Established in May 2016 and transformed from Shanghai Songjiang Ledu Hospital set up in May 1995, Shanghai Fifth Rehabilitation Hospital is a public medical institution with rehabilitation medicine and geriatric medicine as its feature disciplines, and integrating functions of basic medical care and preventive physical examination. Based on rehabilitation informatization, it has established a hierarchical rehabilitation system of three grades from Grade-III facilities, Grade-II facilities to Community Health Center, and in this way has promoted patient transferal among hospitals of various grades in the region.



To enhance service flow construction, constantly improve service quality and achieve optimal patient and employee satisfaction, Ledu Rehabilitation Hospital has carried out comprehensive reform covering both soft environment like hospital management and service process, and hardware environment like medical equipment and architecture, and has consecutively passed three rounds of CARF accreditations.

Soft environment reform covering hospital management and service process based on CARF standards:

1. Formulate new hospital management manual, carry out training and then implement;

2. Improve inpatient and outpatient service flow, adjust and perfect medical record framework, team meeting mode, evaluation mode, follow-up visit system and outcome analysis system.


Hardware environment reform covering hospital architecture and facilities based on CARF standards:

1. Reform hospital traffic system, set up passenger-vehicle separation system and parking lots for the disabled;

2. Reform hospital signage system, and add guidance information of up, middle and low spaces.

3. Humanistic hospital environment construction, newly build Patient Education Room, Activity Room and Patient Voice Board;

4. Medical environment accessibility reform, widen doors of consulting rooms, reconstruct doors of washrooms, build accessible ramps, and add handrail system.
